FALL 2026 POND HOCKEY REGISTRATION
Fall Pond Hockey will run for 12 consecutive weeks, beginning Sunday September 27th, and ending Sunday December 13th. All games played at the West Kelowna Hockey Centre. Please fill out this form to register your player for their Fall season.

If your child would be better placed in an age group different than their own, register them for the most appropriate division and specify in the notes section. We reserve the right to place your child up or down an age group if skill level is better matched in one different than their own.

I submit this Waiver and Usage Agreement on behalf of the minor named above (the “PARTICIPANT”), who will be visiting or participating in an ice program or any private rental at the West Kelowna Hockey Centre (“WKHC”).

I confirm that I am the PARTICIPANT’s legal guardian, am at least nineteen (19) years old, and have read, understood and voluntarily agreed to this Agreement.

I acknowledge that skating, hockey and related activities involve inherent risks, including serious injury, illness, property damage and death. To the fullest extent permitted by law, I release and discharge WKHC and its owners, employees, agents and successors from all claims or liability arising from the PARTICIPANT’s presence at or participation in activities at WKHC, including claims arising from negligence.

I agree that the PARTICIPANT and accompanying guests will:
• Follow all WKHC rules, safety requirements and staff instructions.
• Behave responsibly and refrain from unsafe, disruptive, abusive or destructive conduct.
• Wear all equipment required for their activity, including a CSA-approved helmet, while on the ice or player benches.
• Remain off the ice until the scheduled start time and leave promptly at the end of the session.
• Remain off the ice during resurfacing until the Zamboni has left and the arena gates are closed.
• Keep dressing rooms and other occupied areas clean and vacate dressing rooms within 20 minutes after the session.
• Stay out of restricted areas and refrain from spitting, sharing water bottles, or bringing food into dressing rooms.
• Refrain from smoking, vaping or consuming alcohol or illegal drugs anywhere on WKHC property.

Children must be supervised by a parent, guardian, designated group representative or authorized WKHC program staff, as applicable. Private rental groups must designate a responsible adult who is at least 19 years old to supervise the group.

The PARTICIPANT or responsible group representative may be charged for damage, excessive cleaning or other costs resulting from the conduct of the PARTICIPANT or anyone associated with their group. WKHC is not responsible for lost, stolen or damaged personal property.

WKHC may refuse entry or remove, suspend or prohibit anyone whose conduct violates WKHC policies or poses a risk to people, property or facility operations.

Ice bookings and program registrations are non-refundable. No refund or credit will be provided for participant cancellations, withdrawals or missed sessions. If WKHC cancels a session or makes a significant schedule change, WKHC will make reasonable efforts to provide a rescheduled session, credit or other appropriate accommodation.
